3 Terms and Definitions
3.1 Quality
Classification of Employee Injury and Death Accidents in Enterprises
The sum of characteristics and features of products, processes or services that meet specified or potential requirements (or needs). 3.2 Quality accidents
JB/T5057-91
In the process of product quality formation, an event that causes a certain degree of loss to the enterprise or user due to quality problems. 4 Classification of quality accidents
Quality accidents are divided into three categories according to the size of the loss: general quality accidents, major quality accidents and serious quality accidents. 4.1 General quality accidents are those that fall into any of the following circumstances: a. For an enterprise with an annual total output value of less than or equal to 10 million yuan, a one-time direct economic loss exceeds 0.023 or 0.1 million yuan of the annual total output value of the enterprise;
b. For an enterprise with an annual total output value of more than 10 million yuan but less than or equal to 50 million yuan, a one-time direct economic loss exceeds 0.01% of the annual total output value of the enterprise or 0.25 million yuan; c. For an enterprise with an annual total output value of more than 50 million yuan, a one-time direct economic loss exceeds 0.005% of the annual total output value of the enterprise or 0.1 million yuan of quality accidents.
4.2 Any of the following circumstances shall be deemed a major quality accident: a. For an enterprise with an annual total output value of less than or equal to RMB 10 million, a one-time direct economic loss exceeds 10% of the enterprise's annual total output value or a quality accident of RMB 5,000; b. For an enterprise with an annual total output value of greater than RMB 10 million but less than or equal to RMB 50 million, a one-time direct economic loss exceeds 0.05% of the enterprise's annual total output value or a quality accident of RMB 12,500; c. For an enterprise with an annual total output value of greater than RMB 50 million, a one-time direct economic loss exceeds 0.025% of the enterprise's annual total output value or a quality accident of RMB 20,000; d. Serious injuries to personnel due to product quality issues. 4.3 Any of the following circumstances shall be deemed as a major quality accident: 1. For an enterprise with an annual total output value of less than or equal to RMB 10 million, a one-time direct economic loss exceeds 0.40% of the annual total output value of the enterprise or a quality accident of RMB 20,000;
Approved by the Ministry of Machinery and Electronics Industry of the People's Republic of China on May 17, 1991 and implemented on July 1, 1992

b. For an enterprise with an annual total output value of more than RMB 10 million but less than or equal to RMB 50 million, a one-time direct economic loss exceeds 0.20% of the annual total output value of the enterprise or a quality accident of RMB 50,000; c. For an enterprise with an annual total output value of more than RMB 50 million, a one-time direct economic loss exceeds 0.10% of the annual total output value of the enterprise or a quality accident of RMB 80,000;
d. Death caused by product quality problems. Note: ① Annual total output value is calculated at current prices:
③ Annual total output value can be replaced by the planned total output value of the year, 4.4 When the product of the annual total output value and the specified proportion exceeds the specified absolute number, it is calculated as the absolute number; when the product of the annual total output value and the specified proportion does not exceed the specified absolute number, it is calculated as the relative number, that is, the smallest of the two numbers shall prevail. For example, an enterprise with an annual total output value of 40 million yuan. Its classification standard for major quality accidents is 50,000 yuan, not 80,000 yuan; for another example, an enterprise with an annual total output value of 20 million yuan, its classification standard for major quality accidents is 40,000 yuan, not 50,000 yuan,: 5 Direct economic losses caused by quality accidents
The direct economic losses caused by quality accidents mainly include the following aspects: process time loss costs. Including the processing time cost and rework time cost before the discovery of the quality accident, a.
Material loss cost, material loss cost is the difference between the material value and the residual value; b.
Medical expenses, transportation expenses, compensation fees, compensation fees, return loss fees, packaging fees, transportation fees and travel expenses of repair personnel caused by casualties; d.
Other expenses.
6 Contents of quality accident information
Quality accident information mainly includes the following contents: e
The time, place and name of the relevant product of the quality accident; the process and cause analysis of the quality accident; the type of quality accident and the size of the direct economic loss; the main responsible department or person;
The handling of the quality accident;
Others,
